Offensively, he possesses power—having hit 30 or more home runs in a season three times—and is a prolific hitter of doubles, leading the league in that category on four occasions. He is characterized by a high batting average and on-base percentage, having posted a batting average of .300 or better eight times. Defensively, he won the Gold Glove Award in 2018.

In NPB, he played for the Pacific League's Orix Buffaloes from 2017 to 2023, contributing to three consecutive league championships (2021–2023) and a Japan Series title in 2022. His career NPB statistics include 172 appearances, a 70–29 record with one save, 922 strikeouts, and a 1.82 ERA. He won 14 major league titles and received 12 major awards—including three each of the Eiji Sawamura Award, Most Valuable Player (MVP), Best Nine, and Gold Glove Award. His combined total of 26 titles and awards ranks second all-time among NPB pitchers, trailing only Tomoyuki Sugano (27). In 2021, he became the eighth player in NPB history—and the first in the Reiwa era—to achieve the pitching "quadruple crown" (leading the league in wins, ERA, strikeouts, and winning percentage). He repeated this feat in both 2022 and 2023—marking the first time in NPB history a pitcher achieved the quadruple crown for two and three consecutive years—while also tying the all-time record for the most Sawamura Awards (three) and Pacific League MVP awards (three), including tying the record for three consecutive wins of each. Starting in 2024, he pursued a career in MLB with the Los Angeles Dodgers. In his debut season, he helped the team win the National League West, the League Championship Series, and the World Series. During the 2025 World Series, he recorded three wins—a feat not accomplished since Randy Johnson in 2001—and contributed to the Dodgers' first-ever back-to-back World Series championships. He was named World Series MVP, becoming the second Japanese player to receive the honor, following Hideki Matsui in 2009.

He is a "two-way player"—a rarity in modern professional baseball—who performs as both a pitcher and a position player (or designated hitter) throughout the season. He has won the Major League Baseball (MLB) Most Valuable Player (MVP) award four times in his career, the second-highest total in history. He has won the award four times in the last five seasons—including three consecutive years—and achieved this feat unanimously on every occasion. He is the first player in modern MLB history to qualify for both the pitching and hitting leaderboards (meeting the minimum requirements for innings pitched and plate appearances) in a single season. He achieved the feat of recording double-digit wins and double-digit home runs in the same season in both MLB and NPB. He won the NPB MVP award once and became the first Asian player in MLB to lead the league in home runs (twice) and RBIs (once). He also became the first player in MLB history to achieve a 50-home run, 50-stolen base season (joining the "50-50 club"). He holds the record for the most consecutive strikeouts (13 batters)—a mark that stands as both a Nippon Professional Baseball (NPB) and world record—as well as the record for the most strikeouts in a single game (19), tying the NPB record. He is the youngest player in NPB history to throw a perfect game in an official match (Roki Sasaki's perfect game); he was the first player born in the Heisei era and the 21st century to achieve this feat, the first to do so in both the 21st century and the Reiwa era, and remains the only active professional player (as of 2025) to have accomplished it. He is known as the "Monster of the Reiwa Era."
